Here are some forms to help you make an informed decision when hiring a contractor. Pay special attention to the Contractor Questionnaire because it will arm you with all of the most important questions you need answered when interviewing a contractor.

Contractor Questionnaire This is a set of important questions to ask prospective contractors with helpful notes applicable to each question. Almost as important as the answers themselves is how they answer the questions. If they don’t know, make excuses or deflect then you know that there is an issue. Any qualified, licensed and insured contractor should be able to easily answer all of the questions and provide a copy of their license and insurance during the initial meeting when they are providing a quote.

Pay special attention to the name on the license, year issued, classifications, who is the qualifying individual and confirm worker’s comp insurance. If you see a complaint against the license then you know to look no further.

Shorebreak 2023 Insurance Certificate This is what an insurance certificate should look like when provided by a contractor. Pay special attention to the coverage limits and make sure that there is coverage for general liability and worker’s comp. If they are missing either than look no further.

Remember that the price on a quote doesn’t mean anything if they aren’t operating legally and can’t provide the services and products in the quote.
